Position Summary/About the role
Schedule: 12-hour shifts, working full time - Night Shift- 6:30pm-6:00am (40 hours per week).
Administer medication and perform treatments on an assigned unit as ordered by the physician and within scope of license. (Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. and treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care.)
Observe and report the effectiveness of medications and treatments
Ensure an adequate amount of supplies and medication are available
Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs
Participate in the receiving and giving shift reports
Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team
Complete the initial admission nursing assessment tool on a newly admitted resident
Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans
Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress.
Participate in our Quality Assurance and Performance Improvement (QAPI) program
Attend and participate in staff meetings Knowledge of:
Gerontological/Geriatric nursing theory and practice
Effects of the aging process Practices and procedures of team nursing
Practices and procedures for implementing treatment plans for resident/patient care
Practices and procedures of pharmacology and medicines Federal, State and agency laws, rules and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing thereof Skills in:
Observing and reporting behavior and progress of residents/patients Applying and interpreting State and agency laws and regulations governing geriatric nursing practices.
Assisting in the development and implementation of patient care plans within a team nursing concept
Oral and written communication skills Interpersonal skills
Ability to:
Abide by nursing regulations
Listen, observe, assess and respond to medical emergencies
Evaluate, analyze, make independent decisions and follow up regarding client health problems
Respond to medical issues/inquiries in a professional manner
Establish and maintain effective work relationships
