DESCRIPTION
OT, Home Health – Orange County Position Overview: This is a full-time, field-based home health role covering one assigned county in the state of FL. Clinicians will provide care to patients across multiple agency partners and document within various EMR systems. - Full-time: 30 visits per week (adjusted for SOC/OASIS documentation) with a 35 hour guarantee - Average: 5–6 patient visits per day Scheduling & Territory: - Daily schedules finalized by 5:00 PM the day prior - Clinicians coordinate exact visit times directly with patients - Supported by a dedicated scheduling team for updates and adjustments Weekend Need: - Saturday coverage (Orthopedic visits) strongly preferred - Flexible 5-day schedule (example: Tuesday–Saturday) Clinical Structure: - PTs focus on higher-level visits (SOC, Evaluations, Reassessments, Discharges) - PTAs assist with routine visits when appropriate - Opportunity to work across multiple home health agency environments Technology & Documentation: - Central scheduling platform: Pariox EMR (single source for full schedule visibility) - Documentation completed within partner agency EMRs, including: Kantime Kinnser (WellSky) Axxess Homecare Homebase Work Environment: - Home Health - Virtual onboarding and training - Strong administrative and scheduling support - Flexible, autonomous field-based role Additional Facility Information: - Axiom Rehab, LLC is a Florida-based rehabilitation provider delivering Physical, Occupational, and Speech Therapy services within the home health setting. With over 20 years of experience, Axiom Rehab has grown from its roots in Altamonte Springs to supporting home health agencies across nearly the entire state of Florida. - Axiom Rehab operates as a licensed healthcare services pool, partnering with home health agencies to provide therapy services. This unique model allows clinicians to work with multiple agency partners while benefiting from consistent scheduling, administrative support, and a steady patient caseload. - They specialize exclusively in the geriatric population, focusing on helping patients improve mobility, maintain independence, and safely remain in their homes.
